Booking Terms and Conditions
The Terms and Conditions as set out below are integral to the contract of services between the assessor, Alison Bradshaw, and the client.
Payment:
The assessment fee is available on request. Upon receipt of a booking form and deposit, an invoice will be issued and an assessment time/date will be confirmed by mutual agreement.
1. Payment must be made within 7 days from receipt of the invoice, unless otherwise specified.
2. A non-refundable deposit of £50 is required in advance of any bookings in order to secure the booking, unless otherwise specified.
3. Full payment of the outstanding amount must be paid at least 7 days prior to the agreed assessment date.
4. All expenses for training and tutoring materials will be covered in your agreed fee with Alison Bradshaw, unless otherwise specified. All prices are exclusive of mileage, unless otherwise specified. Mileage is charged at a rate of £0.45p per mile.
5. An assessment will include a full battery of relevant tests and a full diagnostic report. The arrangement and fee is for one such assessment, no part of the assessment may be separated off as an individual item.
Pre-Assessment Requirements:
1. It is essential that the assessee has had an eyesight check within 6 months prior to the assessment. Glasses must be brought to the assessment if required. Hearing must also be checked should there be any indication of difficulty prior to the assessment.
2. Before the diagnostic assessment, questionnaires will be sent to you in order to provide essential background information which is used to inform the assessment. Follow up phone calls may also be made to support this information. Completion of both the parents' questionnaire and school questionnaire is mandatory prior to the assessment and failure to provide these documents may result in the cancellation of the assessment.
3. The assessor retains the right to cancel or reschedule the assessment if the aforementioned documents are not made available.
During the Assessment
1. The assessment will be carried out at an agreed venue.
2. An adult known by the learner, for example the learner’s parent or carer, is asked to be present in a room nearby for the duration of the assessment, or to leave an emergency contact number with the assessor, as mutually agreed.
3. Learners under 18 will under no circumstances be left unattended by the assessor or any unknown adult during tuition or assessment.
4. Appointment times should be adhered to. In the event of appointments not being attended on time the session will end at the pre-arranged time, and charged at the same rate.
6. The assessor will act with professionalism during the assessment. The assessee is required to behave in a manner conducive to determining an accurate diagnosis. Where co-occurring needs mean that extra accommodations may be needed, this should be discussed with the assessor prior to assessment.
7. The assessor reserves the right to pause or postpone an assessment where an individual is becoming unduly upset by the process.
Following the Assessment:
1. The report will be completed within 21 working days following the assessment and will be supplied by email as a password protected PDF. This will be written to the required standard and adhere to the guidelines outlined by The SpLD Assessment Standards Committee (SASC). It will be sent to the client who is then advised to share it with the school/parent. The assessor will be available to discuss the report at a time arranged. No extra fee will be charged for this discussion.
2. If necessary, further follow up tests may be needed. Time and venue for these will be arranged after the initial testing. No further charge will be made for these tests.
3. If the assessee shows signs of a visual difficulty during the assessment, the assessor reserves the right to withhold an assessment report until the extent/impact of any visual difficulties has been determined.
4. Minor amendments to the background information may be requested up to 30 days after a report has been received.
5. Once a report has been finalised (no later than 30 days after the report has been submitted), this concludes the assessment process.
6. Please note: the report will not be issued to the client until payment has been made in full.
Cancellation policy:
Cancellation by the Client
1. All changes and cancellations must be notified via email to alisonbradshaw59@gmail.com or telephone 07833 950002.
2. A full refund will be given if the cancellation date is 14 days or more before the scheduled event, minus deposit. The assessor will always endeavour to reschedule although this may mean a significant wait until the next available appointment. The deposit will need to be paid again.
3. If cancellation is received 7 days or less before the scheduled assessment, the client will be charged 50% of the agreed fee.
4. If cancellation is received less than 24 hours before the scheduled assessment, the full assessment fee will be charged.
5. The full fee is payable if you do not cancel your booking and do not attend your booking.
6. Please note a discretionary administration fee of £40 may be made if you request to transfer your booking to a different date less than 7 days prior to the booking date.
Cancellation by the Assessor
In the event that severe weather, illness or any other emergency results in the cancellation of a session, the assessor will endeavour to give clients a minimum of 24 hours’ notice and to reschedule the assessment as soon as possible. A full refund will be issued if the new dates are not suitable.
Non-Refundable Costs
The client will be liable to meet any non-refundable costs associated with the booking which cannot be cancelled regardless of when the assessor is notified of the cancellation.

If you have any concerns about any practice or incident relating to the assessor Alison Bradshaw, please contact Alison in the first instance for an informal discussion. If this does not solve your concern, you may refer to the PATOSS complaints procedure.
DBS and Indemnity Insurance:
Alison Bradshaw has full DBS and indemnity insurance for working with vulnerable individuals.
General:
Alison Bradshaw reserves the right to alter these terms and conditions. Alison will, however, notify you of any changes. Unless otherwise agreed in writing, these Terms and Conditions shall prevail over any other terms of business or conditions put forward.
